CPC is a private, non-profit social service agency serving the Chinese immigrant population in Manhattan, Queens and Brooklyn<\/p>\n
Summary of Position: <\/span><\/strong><\/p>\n Through the development of trusting relationships, assist individuals with developmental disabilities gain access to necessary services and supports appropriate to the needs of the individual using a person centered planning process.<\/p>\n Responsibilities and Duties<\/span><\/strong><\/p>\n Develop, implement and maintain Individual Service Plans guided by the individual\u2019s Valued Outcomes through regular meetings including home visits.<\/p>\n Liaise with Office of People with Developmental Disabilities to establish and maintain eligibility for services including Medicaid.<\/p>\n Partner with community organizations to provide services to individual and their families.<\/p>\n Advocate for individual to receive community resources and services.<\/p>\n Other duties as assigned.<\/p>\n Required Skills<\/span><\/strong><\/p>\n Attention to detail.<\/p>\n Ability to organize multiple forms and deadlines.<\/p>\n Effective communication.<\/p>\n Qualifications<\/span><\/strong><\/p>\n Associates\/BA\/BS degree in health, human services, social work or related field.<\/p>\n One year experience in developmental disabilities and\/or service coordination.<\/p>\n Fluency in English and Chinese required.<\/p>\n
