Job Description
As Premier’s Bilingual Chinese Service Coordinator, you will be responsible to support the Care Team in managing the coordination and continuity of care by providing member services/customer service telephonically. This includes arranging for contracted services according to care plans with specific responsibility to arrange home care, transportation, medical appointments, durable medical equipment and home-delivered meals. This position also supports enrollment with information and administrative support for referrals and submission of required documentation.
The right candidate will possess a HS Diploma or equivalent, excellent communcation skills, one year experience in a Home care, long term or acute care setting. Bilingual a plus – Spanish, Chinese Russian or Korean. Excellent customer service skills required.
